70ccdb300b Kernel: Panic if the init process dies
If init crashes, all other userspace processes exit too, thus rendering
the system unusable. Previously, the kernel would still keep running
even without a userland, showing just a black screen without any
indication of the issue.

We now panic the kernel, which shows a message on the console. In the
case of the CI runners, it shuts down the virtual machine, so we don't
have to wait for the 1 hour timeout if an issue arises with
SystemServer.

585054d68b Kernel: Comment the living daylights out of signal trampoline/sigreturn
Mere mortals like myself cannot understand more than two lines of
assembly without a million comments explaining what's happening, so do
that and make sure no one has to go on a wild stack state chase when
hacking on these.

cf63447044 Kernel: Move signal handlers from being thread state to process state
POSIX requires that sigaction() and friends set a _process-wide_ signal
handler, so move signal handlers and flags inside Process.
This also fixes a "pid/tid confusion" FIXME, as we can now send the
signal to the process and let that decide which thread should get the
signal (which is the thread with tid==pid, but that's now the Process's
problem).
Note that each thread still retains its signal mask, as that is local to
each thread.

e28af4a2fc Kernel: Stop using HashMap in Mutex
This commit removes the usage of HashMap in Mutex, thereby making Mutex
be allocation-free.

In order to achieve this several simplifications were made to Mutex,
removing unused code-paths and extra VERIFYs:
 * We no longer support 'upgrading' a shared lock holder to an
   exclusive holder when it is the only shared holder and it did not
   unlock the lock before relocking it as exclusive. NOTE: Unlike the
   rest of these changes, this scenario is not VERIFY-able in an
   allocation-free way, as a result the new LOCK_SHARED_UPGRADE_DEBUG
   debug flag was added, this flag lets Mutex allocate in order to
   detect such cases when debugging a deadlock.
 * We no longer support checking if a Mutex is locked by the current
   thread when the Mutex was not locked exclusively, the shared version
   of this check was not used anywhere.
 * We no longer support force unlocking/relocking a Mutex if the Mutex
   was not locked exclusively, the shared version of these functions
   was not used anywhere.

31c1094577 Kernel: Don't mess with thread state in Process::do_exec()
We were marking the execing thread as Runnable near the end of
Process::do_exec().

This was necessary for exec in processes that had never been scheduled
yet, which is a specific edge case that only applies to the very first
userspace process (normally SystemServer). At this point, such threads
are in the Invalid state.

In the common case (normal userspace-initiated exec), making the current
thread Runnable meant that we switched away from its current state:
Running. As the thread is indeed running, that's a bogus change!
This created a short time window in which the thread state was bogus,
and any attempt to block the thread would panic the kernel (due to a
bogus thread state in Thread::block() leading to VERIFY_NOT_REACHED().)

Fix this by not touching the thread state in Process::do_exec()
and instead make the first userspace thread Runnable directly after
calling Process::exec() on it in try_create_userspace_process().

It's unfortunate that exec() can be called both on the current thread,
and on a new thread that has never been scheduled. It would be good to
not have the latter edge case, but fixing that will require larger
architectural changes outside the scope of this fix.

0e72b04e7d Kernel: Perform exec-into-new-image directly in sys$execve()
This ensures that everything allocated on the stack in Process::exec()
gets cleaned up. We had a few leaks related to the parsing of shebang
(#!) executables that get fixed by this.

018dc4bb5c Kernel: Add verification promise violations are propagated properly
This change adds a thread member variable to track if we have a pending
promise violation on a kernel thread. This ensures that all code
properly propagates promise violations up to the syscall handler.

54b9a4ec1e Kernel: Handle promise violations in the syscall handler
Previously we would crash the process immediately when a promise
violation was found during a syscall. This is error prone, as we
don't unwind the stack. This means that in certain cases we can
leak resources, like an OwnPtr / RefPtr tracked on the stack. Or
even leak a lock acquired in a ScopeLockLocker.

To remedy this situation we move the promise violation handling to
the syscall handler, right before we return to user space. This
allows the code to follow the normal unwind path, and grantees
there is no longer any cleanup that needs to occur.

The Process::require_promise() and Process::require_no_promises()
functions were modified to return ErrorOr<void> so we enforce that
the errors are always propagated by the caller.

bc518e39bf Kernel: Make perfcore files owned by UID=0, GID=0
Since perfcore files can be generated during process finalization,
we can't just allow them to contain sensitive kernel information
if they're gonna be owned by the process's own UID+GID.

So instead, perfcores are now owned by 0:0. This is not the most
ergonomic solution, but I'm not sure what we could do to make it nicer.
We'll have to think more about that. In the meantime, this patches up
a kernel info leak. :^)

88b6428c25 AK: Make Vector::try_* functions return ErrorOr<void>
Instead of signalling allocation failure with a bool return value
(false), we now use ErrorOr<void> and return ENOMEM as appropriate.
This allows us to use TRY() and MUST() with Vector. :^)

79fa9765ca Kernel: Replace KResult and KResultOr<T> with Error and ErrorOr<T>
We now use AK::Error and AK::ErrorOr<T> in both kernel and userspace!
This was a slightly tedious refactoring that took a long time, so it's
not unlikely that some bugs crept in.

Nevertheless, it does pass basic functionality testing, and it's just
real nice to finally see the same pattern in all contexts. :^)

aee4786d8e Kernel: Introduce the DeviceManagement singleton
This singleton simplifies many aspects that we struggled with before:
1. There's no need to make derived classes of Device expose the
constructor as public anymore. The singleton is a friend of them, so he
can call the constructor. This solves the issue with try_create_device
helper neatly, hopefully for good.
2. Getting a reference of the NullDevice is now being done from this
singleton, which means that NullDevice no longer needs to use its own
singleton, and we can apply the try_create_device helper on it too :)
3. We can now defer registration completely after the Device constructor
which means the Device constructor is merely assigning the major and
minor numbers of the Device, and the try_create_device helper ensures it
calls the after_inserting method immediately after construction. This
creates a great opportunity to make registration more OOM-safe.

04ba31b8c5 Kernel+Userland: Remove loadable kernel moduless
These interfaces are broken for about 9 months, maybe longer than that.
At this point, this is just a dead code nobody tests or tries to use, so
let's remove it instead of keeping a stale code just for the sake of
keeping it and hoping someone will fix it.

To better justify this, I read that OpenBSD removed loadable kernel
modules in 5.7 release (2014), mainly for the same reason we do -
nobody used it so they had no good reason to maintain it.
Still, OpenBSD had LKMs being effectively working, which is not the
current state in our project for a long time.
An arguably better approach to minimize the Kernel image size is to
allow dropping drivers and features while compiling a new image.

dd82f68326 Kernel: Use KString all the way in sys$execve()
This patch converts all the usage of AK::String around sys$execve() to
using KString instead, allowing us to catch and propagate OOM errors.

It also required changing the kernel CommandLine helper class to return
a vector of KString for the userspace init program arguments.

300402cc14 Kernel: Make it possible for KBufferBuilder creation to fail
This patch adds KBufferBuilder::try_create() and treats it like anything
else that can fail. And so, failure to allocate the initial internal
buffer of the builder will now propagate an ENOMEM to the caller. :^)

7981422500 Kernel: Make Threads always have a name
We previously allowed Thread to exist in a state where its m_name was
null, and had to work around that in various places.

This patch removes that possibility and forces those who would create a
thread (or change the name of one) to provide a NonnullOwnPtr<KString>
with the name.

75564b4a5f Kernel: Make kernel region allocators return KResultOr<NOP<Region>>
This expands the reach of error propagation greatly throughout the
kernel. Sadly, it also exposes the fact that we're allocating (and
doing other fallible things) in constructors all over the place.

This patch doesn't attempt to address that of course. That's work for
our future selves.

a9204510a4 Kernel: Make file description lookup return KResultOr
Instead of checking it at every call site (to generate EBADF), we make
file_description(fd) return a KResultOr<NonnullRefPtr<FileDescription>>.

This allows us to wrap all the calls in TRY(). :^)

The only place that got a little bit messier from this is sys$mount(),
and there's a whole bunch of things there in need of cleanup.

This function is currently only ever used to create the init process
(SystemServer). It had a few idiosyncratic things about it that this
patch cleans up:

- Errors were returned in an int& out-param.
- It had a path for non-0 process PIDs which was never taken.

ba1a6ca971 Kernel: Move pledge verification into Process member functions
REQUIRE_PROMISE and REQUIRE_NO_PROMISES were macros for some reason,
and used all over the place.

This patch adds require_promise(Pledge) and require_no_promises()
to Process and makes the macros call these on the current process
instead of inlining code everywhere.

ae197deb6b Kernel: Strongly typed user & group ID's
Prior to this change, both uid_t and gid_t were typedef'ed to `u32`.
This made it easy to use them interchangeably. Let's not allow that.

This patch adds UserID and GroupID using the AK::DistinctNumeric
mechanism we've already been employing for pid_t/ProcessID.

dea62fe93c Kernel: Guard the all processes list with a Spinlock rather than a Mutex
There are callers of processes().with or processes().for_each that
require interrupts to be disabled. Taking a Mutexe with interrupts
disabled is a recipe for deadlock, so convert this to a Spinlock.

cf271183b4 Kernel: Make Process::current() return a Process& instead of Process*
This has several benefits:
1) We no longer just blindly derefence a null pointer in various places
2) We will get nicer runtime error messages if the current process does
turn out to be null in the call location
3) GCC no longer complains about possible nullptr dereferences when
compiling without KUBSAN

18f260b78b Kernel: Handle removal of Process from list before unref
This makes the following scenario impossible with an SMP setup:

1) CPU A enters unref() and decrements the link count to 0.
2) CPU B sees the process in the process list and ref()s it.
3) CPU A removes the process from the list and continues destructing.
4) CPU B is now holding a destructed Process object.

By holding the process list lock before doing anything with it, we
ensure that other CPUs can't find this process in the middle of it being
destructed.

2830a0ecda Kernel: Move ProcFS related overrides in Process to ProcessProcFSTraits
This allows us to 1) let go of the Process when an inode is ref'ing for
ProcFSExposedComponent related reasons, and 2) change our ref/unref
implementation.

7676edfb9b Kernel: Stop allowing implicit conversion from KResult to int
This patch removes KResult::operator int() and deals with the fallout.
This forces a lot of code to be more explicit in its handling of errors,
greatly improving readability.

40a942d28b Kernel: Remove char* versions of path argument / kstring copy methods
The only two paths for copying strings in the kernel should be going
through the existing Userspace<char const*>, or StringArgument methods.

Lets enforce this by removing the option for using the raw cstring APIs
that were previously available.

01b79910b3 Kernel/Process: Move protected values to the end of the object
The compiler can re-order the structure (class) members if that's
necessary, so if we make Process to inherit from ProcFSExposedComponent,
even if the declaration is to inherit first from ProcessBase, then from
ProcFSExposedComponent and last from Weakable<Process>, the members of
class ProcFSExposedComponent (including the Ref-counted parts) are the
first members of the Process class.

This problem made it impossible to safely use the current toggling
method with the write-protection bit on the ProcessBase members, so
instead of inheriting from it, we make its members the last ones in the
Process class so we can safely locate and modify the corresponding page
write protection bit of these values.

We make sure that the Process class doesn't expand beyond 8192 bytes and
the protected values are always aligned on a page boundary.
