3c9bd911b8 Kernel: Make /proc/PID directories owned by the EUID:EGID
This is instead of the UID:GID, since that was allowing some very bad
information leaks like spawning "su" as an unprivileged user and having
full /proc access to it.

Work towards #4504.

b156c5a8eb ProcFS: pid_vm: Replace duplicated purgeable key with kernel+cacheable
ProcFS /proc/<pid>/vm map info no longer contains two `purgeable` keys.

The second `purgeable` key has been removed and replaced with keys for
`kernel` and `cacheable`.

5f51d85184 Kernel: Improve time keeping and dramatically reduce interrupt load
This implements a number of changes related to time:
* If a HPET is present, it is now used only as a system timer, unless
  the Local APIC timer is used (in which case the HPET timer will not
  trigger any interrupts at all).
* If a HPET is present, the current time can now be as accurate as the
  chip can be, independently from the system timer. We now query the
  HPET main counter for the current time in CPU #0's system timer
  interrupt, and use that as a base line. If a high precision time is
  queried, that base line is used in combination with quering the HPET
  timer directly, which should give a much more accurate time stamp at
  the expense of more overhead. For faster time stamps, the more coarse
  value based on the last interrupt will be returned. This also means
  that any missed interrupts should not cause the time to drift.
* The default system interrupt rate is reduced to about 250 per second.
* Fix calculation of Thread CPU usage by using the amount of ticks they
  used rather than the number of times a context switch happened.
* Implement CLOCK_REALTIME_COARSE and CLOCK_MONOTONIC_COARSE and use it
  for most cases where precise timestamps are not needed.

Everywhere: Switch from (void) to [[maybe_unused]] (#4473)
Problem:
- `(void)` simply casts the expression to void. This is understood to
  indicate that it is ignored, but this is really a compiler trick to
  get the compiler to not generate a warning.

Solution:
- Use the `[[maybe_unused]]` attribute to indicate the value is unused.

Note:
- Functions taking a `(void)` argument list have also been changed to
  `()` because this is not needed and shows up in the same grep
  command.

da5cc34ebb Kernel: Fix some issues related to fixes and block conditions
Fix some problems with join blocks where the joining thread block
condition was added twice, which lead to a crash when trying to
unblock that condition a second time.

Deferred block condition evaluation by File objects were also not
properly keeping the File object alive, which lead to some random
crashes and corruption problems.

Other problems were caused by the fact that the Queued state didn't
handle signals/interruptions consistently. To solve these issues we
remove this state entirely, along with Thread::wait_on and change
the WaitQueue into a BlockCondition instead.

Also, deliver signals even if there isn't going to be a context switch
to another thread.

Fixes #4336 and #4330

046d6855f5 Kernel: Move block condition evaluation out of the Scheduler
This makes the Scheduler a lot leaner by not having to evaluate
block conditions every time it is invoked. Instead evaluate them as
the states change, and unblock threads at that point.

This also implements some more waitid/waitpid/wait features and
behavior. For example, WUNTRACED and WNOWAIT are now supported. And
wait will now not return EINTR when SIGCHLD is delivered at the
same time.

6cb640eeba Kernel: Move some time related code from Scheduler into TimeManagement
Use the TimerQueue to expire blocking operations, which is one less thing
the Scheduler needs to check on every iteration.

Also, add a BlockTimeout class that will automatically handle relative or
absolute timeouts as well as overriding timeouts (e.g. socket timeouts)
more consistently.

Also, rework the TimerQueue class to be able to fire events from
any processor, which requires Timer to be RefCounted. Also allow
creating id-less timers for use by blocking operations.

5f2f31861c Kernel: Use a doubly-linked list for the BlockBasedFS cache
This makes misses in the BlockBasedFS's LRU block cache faster by
storing the cache entries in one of two doubly-linked list.

Dirty and clean cache entries are kept in two separate lists, and
move between them when their state changes. This can probably be
improved upon further.

3e3a72f2a2 Ext2FS: Oops, fix forgotten assignment in Ext2FSInode::resize()
If the inode's block list cache is empty, we forgot to assign the
result of computing the block list. The fact that this worked anyway
makes me wonder when we actually don't have a cache..

a6a3c20071 Kernel: Add a fast lookup table to the BlockBasedFS disk cache
Instead of doing a linear scan of the entire cache when doing a lookup,
we now have a nice O(1) HashMap in front of the cache.

The cache miss case can still be improved, this patch really only helps
the cache hit case.

This dramatically improves cached filesystem I/O. :^)

Since we're using UserOrKernelBuffers, SMAP will be automatically
disabled when we actually access the buffer later on. There's no need
to disable it wholesale across the entire read/write operations.

df758a5a51 Ext2FS: Clear out the direct block list when an inode is resized to 0
e2fsck was complaining about blocks being allocated in an inode's list
of direct blocks while at the same time being free in the block bitmap.

It was easy to reproduce by creating a file with non-zero length and
then truncating it. This fixes the issue by clearing out the direct
block list when resizing a file to 0.

75f61fe3d9 AK: Make RefPtr, NonnullRefPtr, WeakPtr thread safe
This makes most operations thread safe, especially so that they
can safely be used in the Kernel. This includes obtaining a strong
reference from a weak reference, which now requires an explicit
call to WeakPtr::strong_ref(). Another major change is that
Weakable::make_weak_ref() may require the explicit target type.
Previously we used reinterpret_cast in WeakPtr, assuming that it
can be properly converted. But WeakPtr does not necessarily have
the knowledge to be able to do this. Instead, we now ask the class
itself to deliver a WeakPtr to the type that we want.

Also, WeakLink is no longer specific to a target type. The reason
for this is that we want to be able to safely convert e.g. WeakPtr<T>
to WeakPtr<U>, and before this we just reinterpret_cast the internal
WeakLink<T> to WeakLink<U>, which is a bold assumption that it would
actually produce the correct code. Instead, WeakLink now operates
on just a raw pointer and we only make those constructors/operators
available if we can verify that it can be safely cast.

In order to guarantee thread safety, we now use the least significant
bit in the pointer for locking purposes. This also means that only
properly aligned pointers can be used.

bab24ce34c Ext2FS: Deallocate block list meta blocks when freeing an inode
When computing the list of blocks to deallocate when freeing an inode,
we would stop collecting blocks after reaching the inode's block count.
Since we're getting rid of the inode, we need to also include the meta
blocks used by the on-disk block list itself.

a28f29c82c Kernel+LibC: Don't allow a directory to become a subdirectory of itself
If you try to do this (e.g "mv directory directory"), sys$rename() will
now fail with EDIRINTOSELF.

Dr. POSIX says we should return EINVAL for this, but a custom error
code allows us to print a much more helpful error message when this
problem occurs. :^)

Everywhere: Redundant inline specifier on constexpr functions (#3807)
Problem:
- `constexpr` functions are decorated with the `inline` specifier
  keyword. This is redundant because `constexpr` functions are
  implicitly `inline`.
- [dcl.constexpr], §7.1.5/2 in the C++11 standard): "constexpr
  functions and constexpr constructors are implicitly inline (7.1.2)".

Solution:
- Remove the redundant `inline` keyword.

eeffd5be07 Ext2FS: Fix block allocation ignoring the very last block group
The block group indices are 1-based for some reason. Because of that,
we were forgetting to check in the very last block group when doing
block allocation. This caused block allocation to fail even when the
superblock indicated that we had free blocks.

Fixes #3674.

2cb32f8356 Kernel: Let InodeWatcher track child inode numbers instead of names
First of all, this fixes a dumb info leak where we'd write kernel heap
addresses (StringImpl*) into userspace memory when reading a watcher.

Instead of trying to pass names to userspace, we now simply pass the
child inode index. Nothing in userspace makes use of this yet anyway,
so it's not like we're breaking anything. We'll see how this evolves.

0579a2db34 Kernel: Fix kernel crash in get_dir_entries when buffer too small.
Before e06362de9487806df92cf2360a42d3eed905b6bf this was a sneaky buffer
overflow. BufferStream did not do range checking and continued to write
past the allocated buffer (the size of which was controlled by the
user.)

The issue surfaced after my changes because OutputMemoryStream does
range checking.

Not sure how exploitable that bug was, directory entries are somewhat
controllable by the user but the buffer was on the heap, so exploiting
that should be tough.

c8d9f1b9c9 Kernel: Make copy_to/from_user safe and remove unnecessary checks
Since the CPU already does almost all necessary validation steps
for us, we don't really need to attempt to do this. Doing it
ourselves doesn't really work very reliably, because we'd have to
account for other processors modifying virtual memory, and we'd
have to account for e.g. pages not being able to be allocated
due to insufficient resources.

So change the copy_to/from_user (and associated helper functions)
to use the new safe_memcpy, which will return whether it succeeded
or not. The only manual validation step needed (which the CPU
can't perform for us) is making sure the pointers provided by user
mode aren't pointing to kernel mappings.

To make it easier to read/write from/to either kernel or user mode
data add the UserOrKernelBuffer helper class, which will internally
either use copy_from/to_user or directly memcpy, or pass the data
through directly using a temporary buffer on the stack.

Last but not least we need to keep syscall params trivial as we
need to copy them from/to user mode using copy_from/to_user.

4b66692a55 Kernel: Make Heap implementation reusable, and make kmalloc expandable
Add an ExpandableHeap and switch kmalloc to use it, which allows
for the kmalloc heap to grow as needed.

In order to make heap expansion to work, we keep around a 1 MiB backup
memory region, because creating a region would require space in the
same heap. This means, the heap will grow as soon as the reported
utilization is less than 1 MiB. It will also return memory if an entire
subheap is no longer needed, although that is rarely possible.

33138900de FileSystem: Convert file types to DT_* types at a later stage
A change introduced in 5e01234 made it the resposibility of each
filesystem to have the file types returned from
'traverse_as_directory' match up with the DT_* types.
However, this caused corruption of the Ext2FS file format because
the Ext2FS uses 'traverse_as_directory' internally when manipulating
 the file system. The result was a mixture between EXT2_FT_* and DT_*
file types in the internal Ext2FS structures.

Starting with this commit, the conversion from internal filesystem file
types to the user facing DT_* types happens at a later stage,
in the 'FileDescription::get_dir_entries' function which is directly
used by sys$get_dir_entries.

b6c34c0521 Ext2FS: Make reported file_type values match up with those in dirent
This fixes an issue we had in the git port where git would not
recognize untracked files (for example in 'git status').
When git used readdir, the 'd_type' field in the dirent struct contained
bad values (Specifically, it contained the values defiend in
Kernel/FileSystem/ext2_fs.h instead of the ones in LibC/dirent.h).

After this fix, we can create a new git repository with 'git init', and
then stage and commit files as usual.

d89582880e Kernel: Switch singletons to use new Singleton class
MemoryManager cannot use the Singleton class because
MemoryManager::initialize is called before the global constructors
are run. That caused the Singleton to be re-initialized, causing
it to create another MemoryManager instance.

eeaba41d13 Kernel: Add DirectoryEntryView for VFS directory traversal
Unlike DirectoryEntry (which is used when constructing directories),
DirectoryEntryView does not manage storage for file names. Names are
just StringViews.

This is much more suited to the directory traversal API and makes
it easier to implement this in file system classes since they no
longer need to create temporary name copies while traversing.

c44d2515b4 Kernel: Emit ProcFS pledge/veil properties as "N/A" for kernel processes
Pledges and Veil state don't really make sense for kernel mode
processes, as they can do what ever they want since they are in
kernel mode. Make this clear in the system monitor UI by marking
these entries as null.

The SI prefixes "k", "M", "G" mean "10^3", "10^6", "10^9".
The IEC prefixes "Ki", "Mi", "Gi" mean "2^10", "2^20", "2^30".

Let's use the correct name, at least in code.

Only changes the name of the constants, no other behavior change.

5e48eda218 Kernel: Avoid ninja-import of global variable
This would have caused an issue later when we enable -Wmissing-declarations, as
the compiler didn't see that Kernel::all_inodes() was being used elsewhere, too.
Also, this means that if the type changes later, there's not going to be weird
run-time issues, but rather a nice type error during compile time.

42b057b0c9 Kernel: Mark compilation-unit-only functions as static
This enables a nice warning in case a function becomes dead code. Also, in case
of signal_trampoline_dummy, marking it external (non-static) prevents it from
being 'optimized away', which would lead to surprising and weird linker errors.

I found these places by using -Wmissing-declarations.

The Kernel still shows these issues, which I think are false-positives,
but don't want to touch:

62ec6e5fe0 Kernel: Make Inode::read_entire() return a KBuffer (not ByteBuffer)
ByteBuffer is backed by kmalloc heap memory which is a scarce resource.

b3d1a05261 Refactor: Expose const_cast by removing ByteBuffer::warp(const void*, size_t)
This function did a const_cast internally which made the call side look
"safe". This method is removed completely and call sites are replaced
with ByteBuffer::wrap(const_cast<void*>(data), size) which makes the
behaviour obvious.

We need to have a Thread lock to protect threading related
operations, such as Thread::m_blocker which is used in
Thread::block.

Also, if a Thread::Blocker indicates that it should be
unblocking immediately, don't actually block the Thread
and instead return immediately in Thread::block.

538b985487 Kernel: Remove ProcessInspectionHandle and make Process RefCounted
By making the Process class RefCounted we don't really need
ProcessInspectionHandle anymore. This also fixes some race
conditions where a Process may be deleted while still being
used by ProcFS.

Also make sure to acquire the Process' lock when accessing
regions.

Last but not least, there's no reason why a thread can't be
scheduled while being inspected, though in practice it won't
happen anyway because the scheduler lock is held at the same
time.

fc0ec60d82 ProcFS: JSONify /proc/cpuinfo
To be more in line with other parts of Serenity's procfs, the
"key: value" format of /proc/cpuinfo was replaced with JSON, namely
an array of objects (one for each core).

The available keys remain the same, though "features" has been changed
from a space-separated string to an array of strings.

bc107d0b33 Kernel: Add SMP IPI support
We can now properly initialize all processors without
crashing by sending SMP IPI messages to synchronize memory
between processors.

We now initialize the APs once we have the scheduler running.
This is so that we can process IPI messages from the other
cores.

Also rework interrupt handling a bit so that it's more of a
1:1 mapping. We need to allocate non-sharable interrupts for
IPIs.

This also fixes the occasional hang/crash because all
CPUs now synchronize memory with each other.

df66c28479 Kernel: Deemphasize inode identifiers
These APIs were clearly modeled after Ext2FS internals, and make perfect sense
in Ext2FS context. The new APIs are more generic, and map better to the
semantics exported to the userspace, where inode identifiers only appear in
stat() and readdir() output, but never in any input.

This will also hopefully reduce the potential for races (see commit c44b4d61f3).

Lastly, this makes it way more viable to implement a filesystem that only
synthesizes its inodes lazily when queried, and destroys them when they are no
longer in use. With inode identifiers being used to reference inodes, the only
choice for such a filesystem is to persist any inode it has given out the
identifier for, because it might be queried at any later time. With direct
references to inodes, the filesystem will know when the last reference is
dropped and the inode can be safely destroyed.

937d0be762 Meta: Add a script check the presence of "#pragma once" in header files
.. and make travis run it.

I renamed check-license-headers.sh to check-style.sh and expanded it so
that it now also checks for the presence of "#pragma once" in .h files.

It also checks the presence of a (single) blank line above and below the
"#pragma once" line.

I also added "#pragma once" to all the files that need it: even the ones
we are not check.
I also added/removed blank lines in order to make the script not fail.

I also ran clang-format on the files I modified.

fdb71cdf8f Kernel: Support read-only filesystem mounts
This adds support for MS_RDONLY, a mount flag that tells the kernel to disallow
any attempts to write to the newly mounted filesystem. As this flag is
per-mount, and different mounts of the same filesystems (such as in case of bind
mounts) can have different mutability settings, you have to go though a custody
to find out if the filesystem is mounted read-only, instead of just asking the
filesystem itself whether it's inherently read-only.

This also adds a lot of checks we were previously missing; and moves some of
them to happen after more specific checks (such as regular permission checks).

One outstanding hole in this system is sys$mprotect(PROT_WRITE), as there's no
way we can know if the original file description this region has been mounted
from had been opened through a readonly mount point. Currently, we always allow
such sys$mprotect() calls to succeed, which effectively allows anyone to
circumvent the effect of MS_RDONLY. We should solve this one way or another.

6af2418de7 Kernel: Pass a Custody instead of Inode to VFS methods
VFS no longer deals with inodes in public API, only with custodies and file
descriptions. Talk directly to the file system if you need to operate on a
inode. In most cases you actually want to go though VFS, to get proper
permission check and other niceties. For this to work, you have to provide a
custody, which describes *how* you have opened the inode, not just what the
inode is.

know how this File (more specifically, InodeFile) was opened in order to decide

3a90a01dd4 Ext2FS: Fix indirect-blocks iteration
For singly-indirect blocks, "callback" is just "add_block".
For doubly-indirect blocks, "callback" is the lambda function
iterating on singly-indirect blocks: so instead of adding itself to the
list, the doubly-indirect block will add all its childs, but they add
themselves again when they run the callback of singly-indirect blocks.
And nothing adds the doubly-indirect block itself :(

This leads to a double free of all child blocks of the doubly-indirect
block, which is the failed assert described in #1549.

88e23113ae Kernel: Tweak FileBackedFS API to avoid intermediary copies
read_block() and write_block() now accept the count (how many bytes to read
or write) and offset (where in the block to start; defaults to 0). Using these
new APIs, we can avoid doing copies between intermediary buffers in a lot more
cases. Hopefully this improves performance or something.

9c856811b2 Kernel: Add Region helpers for accessing underlying physical pages
Since a Region is basically a view into a potentially larger VMObject,
it was always necessary to include the Region starting offset when
accessing its underlying physical pages.

Until now, you had to do that manually, but this patch adds a simple
Region::physical_page() for read-only access and a physical_page_slot()
when you want a mutable reference to the RefPtr<PhysicalPage> itself.

A lot of code is simplified by making use of this.

1d468ed6d3 AK: Stop allowing implicit downcast with RefPtr and NonnullRefPtr
We were allowing this dangerous kind of thing:

RefPtr<Base> base;
RefPtr<Derived> derived = base;

This patch changes the {Nonnull,}RefPtr constructors so this is no
longer possible.

To downcast one of these pointers, there is now static_ptr_cast<T>:

RefPtr<Derived> derived = static_ptr_cast<Derived>(base);

Fixing this exposed a ton of cowboy-downcasts in various places,
which we're now forced to fix. :^)

8fbdda5a2d Kernel: Implement basic support for sys$mmap() with MAP_PRIVATE
You can now mmap a file as private and writable, and the changes you
make will only be visible to you.

This works because internally a MAP_PRIVATE region is backed by a
unique PrivateInodeVMObject instead of using the globally shared
SharedInodeVMObject like we always did before. :^)

Linux creates holes in block lists for all-zero content. This is very
reasonable and we can now handle that situation as well.

Note that we're not smart enough to generate these holes ourselves yet,
but now we can at least read from such files.

c44b4d61f3 Kernel: Make Inode::lookup() return a RefPtr<Inode>
Previously this API would return an InodeIdentifier, which meant that
there was a race in path resolution where an inode could be unlinked
in between finding the InodeIdentifier for a path component, and
actually resolving that to an Inode object.

Attaching a test that would quickly trip an assertion before.

f38cfb3562 Kernel: Tidy up debug logging a little bit
When using dbg() in the kernel, the output is automatically prefixed
with [Process(PID:TID)]. This makes it a lot easier to understand which
thread is generating the output.

This patch also cleans up some common logging messages and removes the
now-unnecessary "dbg() << *current << ..." pattern.

0569123ad7 Kernel: Add a basic implementation of unveil()
This syscall is a complement to pledge() and adds the same sort of
incremental relinquishing of capabilities for filesystem access.

The first call to unveil() will "drop a veil" on the process, and from
now on, only unveiled parts of the filesystem are visible to it.

Each call to unveil() specifies a path to either a directory or a file
along with permissions for that path. The permissions are a combination
of the following:

- r: Read access (like the "rpath" promise)
- w: Write access (like the "wpath" promise)
- x: Execute access
- c: Create/remove access (like the "cpath" promise)

Attempts to open a path that has not been unveiled with fail with
ENOENT. If the unveiled path lacks sufficient permissions, it will fail
with EACCES.

Like pledge(), subsequent calls to unveil() with the same path can only
remove permissions, not add them.

Once you call unveil(nullptr, nullptr), the veil is locked, and it's no
longer possible to unveil any more paths for the process, ever.

This concept comes from OpenBSD, and their implementation does various
things differently, I'm sure. This is just a first implementation for
SerenityOS, and we'll keep improving on it as we go. :^)

f4f958f99f Kernel: Make DoubleBuffer use a KBuffer instead of kmalloc()ing
Background: DoubleBuffer is a handy buffer class in the kernel that
allows you to keep writing to it from the "outside" while the "inside"
reads from it. It's used for things like LocalSocket and TTY's.
Internally, it has a read buffer and a write buffer, but the two will
swap places when the read buffer is exhausted (by reading from it.)

Before this patch, it was internally implemented as two Vector<u8>
that we would swap between when the reader side had exhausted the data
in the read buffer. Now instead we preallocate a large KBuffer (64KB*2)
on DoubleBuffer construction and use that throughout its lifetime.

This removes all the kmalloc heap traffic caused by DoubleBuffers :^)

167b57a6b7 TmpFS: Grow the underlying inode buffer with 2x factor when written to
Before this, we would end up in memcpy() churn hell when a program was
doing repeated write() calls to a file in /tmp.

An even better solution will be to only grow the VM allocation of the
underlying buffer and keep using the same physical pages. This would
eliminate all the memcpy() work.

I've benchmarked this using g++ to compile Kernel/Process.cpp.
With these changes, compilation goes from ~35 sec to ~31 sec. :^)

d0d13e2bf5 Kernel: Move setting file flags and r/w mode to VFS::open()
Previously, VFS::open() would only use the passed flags for permission checking
purposes, and Process::sys$open() would set them on the created FileDescription
explicitly. Now, they should be set by VFS::open() on any files being opened,
including files that the kernel opens internally.

This also lets us get rid of the explicit check for whether or not the returned
FileDescription was a preopen fd, and in fact, fixes a bug where a read-only
preopen fd without any other flags would be considered freshly opened (due to
O_RDONLY being indistinguishable from 0) and granted a new set of flags.

ae64fd1b27 Kernel: Let symlinks resolve themselves
Symlink resolution is now a virtual method on an inode,
Inode::resolve_as_symlink(). The default implementation just reads the stored
inode contents, treats them as a path and calls through to VFS::resolve_path().

This will let us support other, magical files that appear to be plain old
symlinks but resolve to something else. This is particularly useful for ProcFS.

ddd0b19281 Kernel: Add a basic chroot() syscall :^)
The chroot() syscall now allows the superuser to isolate a process into
a specific subtree of the filesystem. This is not strictly permanent,
as it is also possible for a superuser to break *out* of a chroot, but
it is a useful mechanism for isolating unprivileged processes.

The VFS now uses the current process's root_directory() as the root for
path resolution purposes. The root directory is stored as an uncached
Custody in the Process object.

9eef39d68a Kernel: Start implementing x86 SMAP support
Supervisor Mode Access Prevention (SMAP) is an x86 CPU feature that
prevents the kernel from accessing userspace memory. With SMAP enabled,
trying to read/write a userspace memory address while in the kernel
will now generate a page fault.

Since it's sometimes necessary to read/write userspace memory, there
are two new instructions that quickly switch the protection on/off:
STAC (disables protection) and CLAC (enables protection.)
These are exposed in kernel code via the stac() and clac() helpers.

There's also a SmapDisabler RAII object that can be used to ensure
that you don't forget to re-enable protection before returning to
userspace code.

THis patch also adds copy_to_user(), copy_from_user() and memset_user()
which are the "correct" way of doing things. These functions allow us
to briefly disable protection for a specific purpose, and then turn it
back on immediately after it's done. Going forward all kernel code
should be moved to using these and all uses of SmapDisabler are to be
considered FIXME's.

Note that we're not realizing the full potential of this feature since
I've used SmapDisabler quite liberally in this initial bring-up patch.

We were running without the sticky bit and mode 777, which meant that
the /tmp directory was world-writable *without* protection.

With this fixed, it's no longer possible for everyone to steal root's
files in /tmp.

In order to ensure a specific owner and mode when the local socket
filesystem endpoint is instantiated, we need to be able to call
fchmod() and fchown() on a socket fd between socket() and bind().

This is because until we call bind(), there is no filesystem inode
for the socket yet.

3be1c7b514 Kernel: Fix awkward bug where "touch /foo/bar/baz" could create "/baz"
To accomodate file creation, path resolution optionally returns the
last valid parent directory seen while traversing the path.

Clients will then interpret "ENOENT, but I have a parent for you" as
meaning that the file doesn't exist, but its immediate parent directory
does. The client then goes ahead and creates a new file.

In the case of "/foo/bar/baz" where there is no "/foo", it would fail
with ENOENT and "/" as the last seen parent directory, causing e.g the
open() syscall to create "/baz".

Covered by test_io.

0a1865ebc6 Kernel: read() and write() should fail with EBADF for wrong mode fd's
It was previously possible to write to read-only file descriptors,
and read from write-only file descriptors.

All FileDescription objects now start out non-readable + non-writable,
and whoever is creating them has to "manually" enable reading/writing
by calling set_readable() and/or set_writable() on them.

e5ffa960d7 Kernel: Create support for PCI ECAM
The new PCI subsystem is initialized during runtime.
PCI::Initializer is supposed to be called during early boot, to
perform a few tests, and initialize the proper configuration space
access mechanism. Kernel boot parameters can be specified by a user to
determine what tests will occur, to aid debugging on problematic
machines.
After that, PCI::Initializer should be dismissed.

PCI::IOAccess is a class that is derived from PCI::Access
class and implements PCI configuration space access mechanism via x86
IO ports.
PCI::MMIOAccess is a class that is derived from PCI::Access
and implements PCI configurtaion space access mechanism via memory
access.

The new PCI subsystem also supports determination of IO/MMIO space
needed by a device by checking a given BAR.
In addition, Every device or component that use the PCI subsystem has
changed to match the last changes.

50677bf806 Kernel: Refactor scheduler to use dynamic thread priorities
Threads now have numeric priorities with a base priority in the 1-99
range.

Whenever a runnable thread is *not* scheduled, its effective priority
is incremented by 1. This is tracked in Thread::m_extra_priority.
The effective priority of a thread is m_priority + m_extra_priority.

When a runnable thread *is* scheduled, its m_extra_priority is reset to
zero and the effective priority returns to base.

This means that lower-priority threads will always eventually get
scheduled to run, once its effective priority becomes high enough to
exceed the base priority of threads "above" it.

The previous values for ThreadPriority (Low, Normal and High) are now
replaced as follows:

    Low -> 10
    Normal -> 30
    High -> 50

In other words, it will take 20 ticks for a "Low" priority thread to
get to "Normal" effective priority, and another 20 to reach "High".

This is not perfect, and I've used some quite naive data structures,
but I think the mechanism will allow us to build various new and
interesting optimizations, and we can figure out better data structures
later on. :^)

Every process keeps its own ELF executable mapped in memory in case we
need to do symbol lookup (for backtraces, etc.)

Until now, it was mapped in a way that made it accessible to the
program, despite the program not having mapped it itself.
I don't really see a need for userspace to have access to this right
now, so let's lock things down a little bit.

This patch makes it inaccessible to userspace and exposes that fact
through /proc/PID/vm (per-region "user_accessible" flag.)

5292f6e78f Kernel+FileManager: Disallow watch_file() in unsupported file systems
Currently only Ext2FS and TmpFS supports InodeWatchers. We now fail
with ENOTSUPP if watch_file() is called on e.g ProcFS.

This fixes an issue with FileManager chewing up all the CPU when /proc
was opened. Watchers don't keep the watched Inode open, and when they
close, the watcher FD will EOF.

Since nothing else kept /proc open in FileManager, the watchers created
for it would EOF immediately, causing a refresh over and over.

Fixes #879.

b32e961a84 Kernel: Implement a simple process time profiler
The kernel now supports basic profiling of all the threads in a process
by calling profiling_enable(pid_t). You finish the profiling by calling
profiling_disable(pid_t).

This all works by recording thread stacks when the timer interrupt
fires and the current thread is in a process being profiled.
Note that symbolication is deferred until profiling_disable() to avoid
adding more noise than necessary to the profile.

A simple "/bin/profile" command is included here that can be used to
start/stop profiling like so:

    $ profile 10 on
    ... wait ...
    $ profile 10 off

After a profile has been recorded, it can be fetched in /proc/profile

There are various limits (or "bugs") on this mechanism at the moment:

- Only one process can be profiled at a time.
- We allocate 8MB for the samples, if you use more space, things will
  not work, and probably break a bit.
- Things will probably fall apart if the profiled process dies during
  profiling, or while extracing /proc/profile

dbb644f20c Kernel: Start implementing purgeable memory support
It's now possible to get purgeable memory by using mmap(MAP_PURGEABLE).
Purgeable memory has a "volatile" flag that can be set using madvise():

- madvise(..., MADV_SET_VOLATILE)
- madvise(..., MADV_SET_NONVOLATILE)

When in the "volatile" state, the kernel may take away the underlying
physical memory pages at any time, without notifying the owner.
This gives you a guilt discount when caching very large things. :^)

Setting a purgeable region to non-volatile will return whether or not
the memory has been taken away by the kernel while being volatile.
Basically, if madvise(..., MADV_SET_NONVOLATILE) returns 1, that means
the memory was purged while volatile, and whatever was in that piece
of memory needs to be reconstructed before use.

Thread backtraces now list the thread name in addtion to tid.

59ed235c85 Kernel: Implement O_DIRECT open() flag to bypass disk caches
Files opened with O_DIRECT will now bypass the disk cache in read/write
operations (though metadata operations will still hit the disk cache.)

This will allow us to test actual disk performance instead of testing
disk *cache* performance, if that's what we want. :^)

There's room for improvment here, we're very aggressively flushing any
dirty cache entries for the specific block before reading/writing that
block. This is done by walking the entire cache, which may be slow.

Don't keep Inodes around in memory forever after we've interacted with
them once. This is a slight performance pessimization when accessing

c1d3ac7108 Ext2FS: Fix unpopulated block list cache after mkdir()
When creating a new directory, we set the initial size to 1 block.
This meant that we were allocating a block up front, but the Inode's
internal block list cache was not populated with this block.

This broke write_bytes() on a new directory, since it assumed that
the block list cache would be up to date if the call to write_bytes()
would not change the directory's size.

This patch fixes the issue in two ways: First, we cache the initial
block list created for new directories.
Second, we now repopulate the block list cache in write_bytes() if it
is empty when we get there. This is basically just a safety fallback
to avoid having this kind of bug in the future.

I have no idea why this was here. It makes no sense. If you're trying
to find out if something is a directory, why wouldn't you be allowed to
ask that about a FIFO? :^)

Also, while we're here, cache the directory state in a bool member so
we don't have to keep fetching inode metadata when checking this
repeatedly. This is important since sys$read() now calls it.

Previously, procfs$pid_fds would return nothing when called
for a process that had either no open files or a non-existent
handle. This could cause problems when a userspace program

1cca5142af Kernel: Make DoubleBuffer use a KBuffer instead of kmalloc()ing
Background: DoubleBuffer is a handy buffer class in the kernel that
allows you to keep writing to it from the "outside" while the "inside"
reads from it. It's used for things like LocalSocket and PTY's.
Internally, it has a read buffer and a write buffer, but the two will
swap places when the read buffer is exhausted (by reading from it.)

Before this patch, it was internally implemented as two Vector<u8>
that we would swap between when the reader side had exhausted the data
in the read buffer. Now instead we preallocate a large KBuffer (64KB*2)
on DoubleBuffer construction and use that throughout its lifetime.

This removes all the kmalloc heap traffic caused by DoubleBuffers :^)

The Cache entries found in `DiskBackedFileSystem` are now stored in a
`KBuffer` object, instead of relying on `kmalloc_eternal`. The number
of entries was exceeding that of the number of bytes allocated to
`kmalloc_eternal`, which in turn caused `mount()` to fail epically
when called.

35138437ef Kernel+SystemMonitor: Add fault counters
This patch adds three separate per-process fault counters:

- Inode faults

    An inode fault happens when we've memory-mapped a file from disk
    and we end up having to load 1 page (4KB) of the file into memory.

- Zero faults

    Memory returned by mmap() is lazily zeroed out. Every time we have
    to zero out 1 page, we count a zero fault.

- CoW faults

    VM objects can be shared by multiple mappings that make their own
    unique copy iff they want to modify it. The typical reason here is
    memory shared between a parent and child process.

a61f6ccc27 Kernel: Implement a simpler, bigger cache for DiskBackedFS
The hashmap cache was ridiculously slow and hurt us more than it helped
us. This patch replaces it with a flat memory cache that keeps up to
10'000 blocks in cache with a simple dirty bit.

The syncd task will wake up periodically and call flush_writes() on all
file systems, which now causes us to traverse the cache and write all
dirty blocks to disk.

There's a ton of room for improvement here, but this itself is already
drastically better when doing repeated GCC invocations.

bff59eff4b Ext2FS: Fix two bugs in block allocation:
1) Off-by-one in block allocation when block size != 1 KB

Due to a quirk in the on-disk layout of ext2, file systems with a block
size of 1 KB have block #1 as their first block, while all others start
on block #0.

2) We had no fallback mechanism when the preferred group was full

We now allocate blocks from the preferred block group as long as it's
possible, and fall back to a simple scan through all groups when the
preferred one is full.

5d491fa1cd Kernel: Add a simple slab allocator for small allocations
This is a freelist allocator with static size classes that works as a
complement to the generic kmalloc(). It's a lot faster than kmalloc()
since allocation just means popping from the freelist.

It's also significantly more compact when there are a lot of objects
smaller than the minimum kmalloc chunk size (32 bytes.)

This patch enables it for the Region and PhysicalPage classes.
In the PhysicalPage (8 bytes) case, it's a huge improvement since we
no longer waste 75% of the storage allocated.

There are also a number of ways this can be improved, so let's keep
working on it going forward.

b9be6b7bb4 Ext2FS: Trying to create a too-long directory entry should ENAMETOOLONG
Also added some assertions to DirectoryEntry in case someone tries to
instantiate them with names that would overflow the name buffer.

DirectoryEntry is a crappy data structure, and the name buffer is also
crappy. Added a FIXME about replacing it with something nicer.

Before this patch, the DirectoryEntry::name buffer would overflow if
you did "touch extremely-long-file-name". Duh.

Fixes #538.

5978993f00 TmpFS: Fix two bugs that broke GCC inside Serenity
- TmpFSInode::write_bytes() needs to allow non-zero offsets
- TmpFSInode::read_bytes() wasn't respecting the offset

GCC puts the temporary files generated during compilation in /tmp,
so this exposed some bugs in TmpFS.

It is now possible to unmount file systems from the VFS via `umount`.
It works via looking up the `fsid` of the filesystem from the `Inode`'s
metatdata so I'm not sure how fragile it is. It seems to work for now

43ce6c5474 Kernel: Move socket role tracking to the Socket class itself
This is more logical and allows us to solve the problem of
non-blocking TCP sockets getting stuck in SocketRole::None.

The only complication is that a single LocalSocket may be shared
between two file descriptions (on the connect and accept sides),
and should have two different roles depending from which side
you look at it. To deal with it, Socket::role() is made a
virtual method that accepts a file description, and LocalSocket
internally tracks which FileDescription is the which one and
returns a correct role.

a96d76fd90 Kernel: Put all VMObjects in an InlineLinkedList instead of a HashTable
Using a HashTable to track "all instances of Foo" is only useful if we
actually need to look up entries by some kind of index. And since they
are HashTable (not HashMap), the pointer *is* the index.

Since we have the pointer, we can just use it directly. Duh.
This increase sizeof(VMObject) by two pointers, but removes a global
table that had an entry for every VMObject, where the cost was higher.
It also avoids all the general hash tabling business when creating or
destroying VMObjects. Generally we should do more of this. :^)

This makes VMObject 8 bytes smaller since we can use the array size as
the page count.

The size() is now also computed from the page count instead of being
a separate value. This makes sizes always be a multiple of PAGE_SIZE,

73c998dbfc Kernel: Refactor TCP/IP stack
This has several significant changes to the networking stack.

* Significant refactoring of the TCP state machine. Right now it's
  probably more fragile than it used to be, but handles quite a lot
  more of the handshake process.
* `TCPSocket` holds a `NetworkAdapter*`, assigned during `connect()` or
  `bind()`, whichever comes first.
* `listen()` is now virtual in `Socket` and intended to be implemented
  in its child classes
* `listen()` no longer works without `bind()` - this is a bit of a
  regression, but listening sockets didn't work at all before, so it's
  not possible to observe the regression.
* A file is exposed at `/proc/net_tcp`, which is a JSON document listing
  the current TCP sockets with a bit of metadata.
* There's an `ETHERNET_VERY_DEBUG` flag for dumping packet's content out
  to `kprintf`. It is, indeed, _very debug_.

79e22acb22 Kernel: Use KBuffers for ProcFS and SynthFS
Instead of generating ByteBuffers and keeping those lying around, have
these filesystems generate KBuffers instead. These are way less spooky
to leave around for a while.

Since FileDescription will keep a generated file buffer around until
userspace has read the whole thing, this prevents trivially exhausting
the kmalloc heap by opening many files in /proc for example.

The code responsible for generating each /proc file is not perfectly
efficient and many of them still use ByteBuffers internally but they
at least go away when we return now. :^)

5ded77df39 Kernel+ProcessManager: Let processes have an icon and show it in the table.
Processes can now have an icon assigned, which is essentially a 16x16 RGBA32
bitmap exposed as a shared buffer ID.

You set the icon ID by calling set_process_icon(int) and the icon ID will be
exposed through /proc/all.

To make this work, I added a mechanism for making shared buffers globally
accessible. For safety reasons, each app seals the icon buffer before making
it global.

Right now the first call to GWindow::set_icon() is what determines the
process icon. We'll probably change this in the future. :^)

This makes assertion failures generate backtraces again. Sorry to everyone
who suffered from the lack of backtraces lately. :^)

We share code with the /proc/PID/stack implementation. You can now get the
current backtrace for a Thread via Thread::backtrace(), and all the traces
for a Process via Process::backtrace().

c8e2bb5605 Kernel: Add a mechanism for listening for changes to an inode.
The syscall is quite simple:

    int watch_file(const char* path, int path_length);

It returns a file descriptor referring to a "InodeWatcher" object in the
kernel. It becomes readable whenever something changes about the inode.

Currently this is implemented by hooking the "metadata dirty bit" in
Inode which isn't perfect, but it's a start. :^)

This prevents us from picking up random headers from ../Root, which may

2bd8118843 Kernel: Change the format of /proc/all to JSON.
Update ProcessManager, top and WSCPUMonitor to handle the new format.

Since the kernel is not allowed to use floating-point math, we now compile
the JSON classes in AK without JsonValue::Type::Double support.
To accomodate large unsigned ints, I added a JsonValue::Type::UnsignedInt.

629501049f Kernel: Fix resolving symlinks in the middle of a path.
If a symlink is not the last part of a path, the remaining part
of the path has to be further resolved against the symlink target.

With this, a path containing a symlink always resolves to the target
of the first (leftmost) symlink in it, for example any path of form
/proc/self/... resolves to the corresponding /proc/pid directory.

75df45d709 Kernel: Fix comparing StringViews with strcmp().
StringView character buffer is not guaranteed to be null-terminated;
in particular it will not be null-terminated when making a substring.
This means that the buffer can not be used with C functions that expect
a null-terminated string. Instead, StringView provides a convinient
operator == for comparing it with Strings and C stirngs, so use that.

This fixes /proc/self/... resolution failures in ProcFS, since the name
("self") passed to ProcFSInode::lookup() would not be null-terminated.

aee9317d86 Kernel: Refactor MemoryManager to use a Bitmap rather than a Vector
This significantly reduces the pressure on the kernel heap when
allocating a lot of pages.

Previously at about 250MB allocated, the free page list would outgrow
the kernel's heap. Given that there is no longer a page list, this does
not happen.

The next barrier will be the kernel memory used by the page records for
in-use memory. This kicks in at about 1GB.

08cd75ac4b Kernel: Rename FileDescriptor to FileDescription.
After reading a bunch of POSIX specs, I've learned that a file descriptor
is the number that refers to a file description, not the description itself.
So this patch renames FileDescriptor to FileDescription, and Process now has
FileDescription* file_description(int fd).

5e1c7cb32c Kernel: Memory-mapped files now have the absolute path as their name.

9ac95d1867 FileSystem: Pass mode_t to Inode::add_child().

8adadf8a46 FileSystem: Reuse existing custodies when possible, and keep them updated.
Walk the custody cache and try to reuse an existing one when possible.
The VFS is responsible for updating them when something happens that would
cause the described relationship to change.

This is definitely not perfect but it does work for the basic scenarios like
renaming and removing directory entries.

e6a8133b0c FileSystem: Merge symlink following logic into path resolution.
When encountering a symlink, we abandon the custody chain we've been working
on and start over with a new one (by recursing into a new resolution call.)
Caching symlinks in the custody model would be incredibly difficult to get
right with all the extra invalidation it would require, so let's just not.

393851418b FileSystem: Port most of the code over to using custodies.
The current working directory is now stored as a custody. Likewise for a
process executable file. This unbreaks /proc/PID/fd which has not been
working since we made the filesystem bigger.

This still needs a bunch of work, for instance when renaming or removing
a file somewhere, we have to update the relevant custody links.

4cb87b1753 FileSystem: Add a Custody class that represents a parent/child guardianship.
A custody is kind of a directory entry abstraction that represents a single
entry in a parent directory that tells us the name of a child inode.

The idea here is for path resolution to produce a chain of custody objects.

Nothing crazy, this just means that PROT_READ allocates readable regions,
and that PROT_WRITE allocates writable ones.
